Pre-Paid Service Plans

We understand that regular foot care is a necessity that many insurance plans do not cover. To offer our patients an opportunity to have quality, regular foot care that fits into their budget, we have established two pre-paid foot care plans.

Payment is due at time of visit. You may pay today by cash, check or credit card (MasterCard, Visa, American Express, Discover).

AFC Plan I: $325.00*

Plan I is limited to patients who have no foot pathology and simply want nail care.

Plan I consists of 6 visits in a 12-month time period (from the time of purchase). It includes an initial exam, medical grade moisturizer and medical grade foor powder.

AFC Plan II: $375.00*

Plan II is for patients who require more specialized time and attention to their feet, and who have no pathology and simply want regular nail care. It is especially designed for patients with especially sensitive feet or reoccuring foot problems.

Plan II consists of 6 visits in a 12-month time period (from the time of purchase). It also includes an initial exam, medical grade moisturizer and medical grade foot powder.

AFC Plan III: $550.00*

Plan III consists of 12 visits in a 12-month time period (from the time of purchase). It also includes an initial exam, medical grade moisturizer and medical grade foot powder.

Please Note: these plans are non-refundable, non-transferable. However, these plans are transferable to a family member in the event of death.
